The first question, what the call should cover, is optional and takes one tap. Ask for a phone number near the end, because people share it once they've decided to be called.

Create a new form
Edit the fields below, then paste to build it.
Create a new hosted form from FormMaker's "Book a consultation" template (template id: consultation-booking).
Fields — edit this list before you send it (add, remove, or change anything):
- What would you like to cover? — multiple choice, optional (choices: A general walkthrough, Pricing and next steps, A specific technical question, Not sure yet — just curious)
- Which company are you with? — short text, required
- What should we call you? — short text, required
- Best number to reach you? — phone, optional
- Where should the invite go? — email, required
If I leave the list blank or ask for different fields, ask me for them in this format, one per line:
"Label — type (short text / long text / email / phone / number / dropdown / multiple choice / checkbox / date) — required or optional"
Make a fresh HubSpot form for it — you don't need an existing one. If any field isn't in my HubSpot yet, create it if you can, otherwise build the rest and tell me what to add. Then give me the share link and the embed code.
Convert an existing form
Reshape a form you already have into this template.
I have an existing HubSpot form I'd like to rebuild as FormMaker's "Book a consultation" template (asked one question at a time). First, list my HubSpot forms so I can tell you which one to use. Then rebuild it in that style — keep my existing fields wherever they fit, and only add what the template needs. When it's live, give me the share link and the embed code.
Create a custom-object form
Point this template at a HubSpot custom object.
Create a form for a HubSpot custom object in the style of FormMaker's "Book a consultation" template (asked one question at a time). If I haven't told you which custom object to use, ask me first — "Which HubSpot custom object would you like to use for this form?" — and wait for my answer before doing anything else. Once I've answered, look up that object's own properties and build the form from those (not contact fields), keeping this template's layout and button. If a property I want doesn't exist yet, tell me — I'll need to add it in HubSpot first. Then give me the share link and the embed code.
