ChatGPT
Add FormMaker to ChatGPT. Needs a paid ChatGPT plan.
You can add FormMaker to ChatGPT and then ask it about your forms. Two things to know first:
- You need a paid ChatGPT plan. This isn't on the free version.
- You only paste a link. There's no separate box for a key or password — your key is already inside the link.
Add FormMaker
- In ChatGPT, open Settings and go to Plugins.
- Click Browse plugins.
- Choose Add a custom connector.
- Type a name — FormMaker — and a short description.
- Paste your link from the dashboard into the box for the server URL. It
looks like
https://mcp.formmaker.co.in/mcp?key=fm_live_…. - Where it asks about signing in, choose No authentication — your key is already in the link, so there's nothing more to add.
- Create it, then switch FormMaker on for your chat.
Can't find Add a custom connector? You may need to switch on Developer mode first — look under Settings → Apps & Connectors → Advanced settings — then go back to Plugins → Browse plugins. ChatGPT moves its menus around now and then, so if a name doesn't match exactly, look for the closest one.

Good to know
If a request that creates or changes a form doesn't go through, your plan may only allow reading. Check the connector's settings in ChatGPT, or use a plan that allows changes.
Your link works like a password — it can open every form in your account. Only add it to your own ChatGPT, never to a shared workspace you don't control, and never anywhere public. If it gets out, make a new one from your dashboard and the old one stops working right away. More about your keys.
